The application includes four primary sections – education, experience, continuing education (for those without an informatics degree), and agreement to follow the AMIA Code of Ethics. To reduce burden on applicants, AMIA will accept unofficial documentation of graduation and the applicant’s attestation of employment for determination of eligibility. Additional information may be requested if questions arise during the application review. Further, as noted below, official transcripts and confirmation from employers will be needed for applications selected for audit. AMIA membership is not required for eligibility for the AHIC exam.
Please note: The name on the AHIC application must exactly match the name listed on your valid, government issued ID. If a name change occurs after submitting an application, please provide the supporting documentation to certification@amia.org.
AHIC Pre-application Working Documents
Please use the checklist and worksheet to help organize your information for the application. Assembling content before you begin is helpful because the application must be completed in one session.
Note: Keep the browser window with the application open. DO NOT navigate away using the back button or other hyperlinks on the page. Data entered will be lost.

Application Audits
