Two health system pharmacists and a former hospitalist will share real-world experiences that illustrate how accurate medication history data (or a lack thereof) makes a significant difference in patient care. Sara M. Panella, PharmD, CPh, BCPS is currently the Manager of Pharmacy Ambulatory Care, Population Health and Transitions of Care for Baptist Health South Florida Pharmacy Enterprise, in Miami, Florida. In her role, she was instrumental in standardizing the medication history and reconciliation process at Baptist Health South Florida.
JoLeigh Sutton, PharmD, BCPS
JoLeigh A. Sutton, PharmD, BCPS is currently a Clinical Pharmacy Specialist in Pharmacy Informatics at Jackson-Madison County General Hospital in Jackson, TN. Dr. Sutton obtained her Doctor of Pharmacy from the McWhorter School of Pharmacy at Sanford University in 2007. After graduation, she completed a Pharmacy Practice Residency at the University of Alabama Hospital in Birmingham, AL. In her role as a Pharmacy clinician and informaticist, she has assisted in the policy creation and implementation of the medication reconciliation process at West Tennessee Healthcare.
Colin Banas, MD, MHA
Colin Banas is an Internal Medicine Hospitalist and former Chief Medical Information Officer for VCU Health System in Richmond, Virginia. He has testified before the U.S. Senate and the Office of the National Coordinator (ONC) on health IT and the Meaningful Use program, and is a former Health IT Fellow for the ONC.
